What is KEPRO's philosophy and approach to managing participants?
Public programs must provide healthcare services with optimal cost-efficiency, often while burdened with treating the aged and the most ill. Health literacy, socioeconomics, access to care, and healthy food all must be considered.
We recognize the uniqueness of each participant’s circumstances, and care management needs. We thoroughly assess each for physical, behavioral, and condition-specific health needs, plus readiness to change. We consider each participant’s socioeconomic status and health literacy to identify barriers that may impede access to care and treatment plan and medication adherence. Our case manager then works to identify plan-covered and community services to eliminate these barriers and improve overall self-care capabilities.
Participants who identify with a person (such as a personal physician, or case manager) have significantly better health outcomes than those who see multiple providers for their health care (De Maeseneer JM, De Prins L, Gosset C, et al. Provider continuity in family medicine: Does it make a difference for total health care costs? Ann Fam Med. 2003;1:144-8.). We assign a registered nurse as each participant’s primary case manager who knows and address all conditions and co-morbidities, and facilitates strong relationship-building with both participants and their providers.
Case managers apply theory-driven research and evidence-based tools and interventions to ensure that cancer patients successfully achieve optimal health outcomes. We accomplish this through active and frequent communication and coordination with the member, member’s family/care giver, member’s provider(s), other vendors, and available employer and/or community resources.
We recognize the invaluable role of family/caregiver support positive health outcomes. So care managers also partner with the loved ones to optimize success.
